The LPN is responsible for providing patient care utilizing the nursing process. The nurse collaborates with the members of the Rehab team and ancillary staff, to provide effective, safe quality care. The LPN is a key member of a team, which promotes patient functional improvement.
1. Demonstrates knowledge of nursing theory and proactive within scope of practice defined for LPN’s by the Kentucky board of Nursing.
1.1 Makes a comprehensive daily assessment on assigned patients as evidence by daily shift report and documentation in the patient’s record.
1.2 Reviews and revises patient care plans according to established policy and procedure.
1.3 Delivers direct patient care based on physicians ‘orders and nursing intervention established on patient care plan as evidenced by documentation in the patient’s record.
1.4 Documents interventions and progress towards patient goals.
1.5 Complete FIMS scale on all patients on admission and discharge.
1.6 Demonstrates knowledge of basic laboratory values by notifying nursing manager and/or physician of any abnormities within shift of lab data. Critical lab results reported within 30 minutes.
1.7 Administers and documents prescribed medications (scheduled/PRN) according to schedule and procedure.
1.8 Performs skin assessment each shift and documents.
1.9 Reviews and ensures implantation of all MD orders each shift.
2. Assists in implementation of appropriate teaching program for individual patients.
2.1 Identifies patient and/or family educational goals and creates worklist item.
2.2 Education is documented on an ongoing basis and completed at least 2 days prior to discharge.2.3 Patient/PCG instructed on home prescriptions and Nursing Discharge Instruction Sheet is complete. (ie DR. name, specialty, and Phone number included.)
3. Communicates effectively with other members of the Nursing Department and with members of the entire interdisciplinary team.
3.1 Reports patient information/status according to guidelines established in policy and procedure to nurse in charge of shift report as indicated by no more than 2 reported incidents of failure to follow shift guidelines.
3.2 Reports patient information/status to immediate supervisor as patient condition changes as indicated by no reported incidents of supervisors not being aware of changes in patient medical status.
3.3 Communicates directly with physicians and other interdisciplinary team members re: changes in patient’s condition, inappropriate patient response to nursing interventions, progress toward achievement of short-and long-term goals, and cancellation of therapy, and other data as indicated by action taken to meet changing patient needs and lack of team members complaints.
3.4 Communicates patient information/status in inter-disciplinary chart rounds forms and attendance as chart rounds as assigned.
3.5 Provides positive, supportive communication to patient family and hospital personnel as demonstrated by supervisor’s observations and no documented complaints.
3.6 Identifies and addresses or notify charge nurse/manager or issues that occur with co-works.
3.7 Identifies and escalates issues to appropriate person if issue not addressed/addressed timely.
4. Exhibits leadership skills to enhance the quality of care delivered on the patient unit.
4.1 Collaborates with nurses in implementing nursing interventions to provide continuity of care as evidenced by documentation of achievement of patient goals in the nurse’s record every shift.
4.2 Delegates specific tasks to ancillary personnel assigned to unit according to their respective job descriptions as indicated by the patient care assignments completed on schedule.
4.3 Assists to orient new personnel according to request to provide specific content and clinical feedback. Demonstrates initiative in performance of nursing care by securing and implementing physicians’ orders and nursing orders without direct instructions of supervisor.

5. As a Certified Athletic Trainer in the outpatient clinic setting, assists in the delivery of treatment to the rehab patient population according to acceptable practice guidelines and in compliance with department policies and procedures.
5.1 Maintain patient care areas in a neat and clean manner according to shift assignment and established policy and procedure.
5.2 Labels patient supplies according to policy and procedure as evidenced by no omissions of supplies being dated and labeled with patient name and room number.
5.3 Maintains charged for such equipment and supplies according to policy and procedure.
5.4 Assures that equipment is properly functioning prior to and during patient use as demonstrated by no use of malfunctioning equipment with patient and no omission of completing maintenance requests.
5.5 Demonstrates effective time management skills as demonstrated by completion of routine patient care assignment within shift to include patient treatments, teaching, documentation and team rounds.
5.6 Follow universal precautions for all patients as evidenced by compliance at all times to facility policy for infection control.
5.7 Plans for patient safety, utilizing safety standards as evidenced at all time with safety procedures in the safety manual.
5.8 Completed monthly Quality Assurance worksheets with no omission of worksheets not being completed.
5.9 Offers assistance to other employees (i.e., nurses, assistants, therapist, care managers, and unit secretaries.
5.10 Wears name tag and locator badge at all times.
6. Demonstrates commitment to professional growth and competence.
6.1 Attends and participates in scheduled shift meetings 80% of the time when on duty and reads and initials all meeting minutes.
6.2 Demonstrates knowledge of current nursing literature and practice by contributing this knowledge in clinical situations.

6.4 Demonstrates a commitment to quality in rehabilitation care as evidenced by successfully passing annually competency in FIMS.
6.5 Consistency completes the FIMS scale on admission and discharge.
6.6 TB skin test obtained as required.
6.7 Completed all mandatory modules/in-services annually.
6.8 Completes competency annually within required time frame.
6.9 Sends certificate of educational programs attended to the education Department.
7. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients serviced.
7.1 Demonstrates the knowledge and skill required to evaluate the appropriateness of medications therapy in geriatric, adolescents, adults, and pediatric patients as documented by participating in continuing education programs.
7.2 Demonstrates the knowledge related to the age-appropriate medication doses of neonates, pediatrics, adolescents, adults, and geriatrics.
7.3 Demonstrates knowledge of dosage forms available to treat pediatric patients.
7.4 Calculates pediatric and adolescent doses.
7.5 Identifies drugs requiring dosage adjustments in geriatric patients. Calculates dosage reductions based on kidney function (CrCI) when appropriate.
