Location: 200 Abraham Flexner Way, Louisville, KY 40202

Shift:  1st Shift, (8 hours, 5 days) M-F

JOB SUMMARY

Nurse Clinician assists the Residents and Interns during their Pediatric Ward Rotation.  The nurse clinician works to streamline discharges by preparing discharge order sets, making follow up appointments, and communicating discharge plans with case manager and families.  The Nurse Clinician may engage in other activities that help the Pediatric Hospitalist Service run smoothly and efficiently.

Responsibilities

JOB RESPONSIBILITIES

Meets with team residents prior to morning report

  • Discusses patient assignment
  • Assists with tasks to be completed prior to rounding, i.e. contacting outlying hospitals, PCPs, radiology and lab to gather data for availability during rounds.
  • Discusses discharge needs of patient assignment and communicates to ancillary staff expediting discharge.

Meets with families proactively assessing needs.

  • Verifies primary care doctor and demographics.
  • Assesses family’s ability to obtain medicine for discharge, initiates prior authorizations.
  • Verifies transportation at D/C.
  • Communicates needs of the patient to case managers and social workers PRN.
  • Informs family about Family Cantered Rounds assists family with questions and rounding process.

Rounds with team

  • Assists with discharge order completion, prescription preparation, and medicine reconciliation forms.
  • Gathers team members for rounds.
  • Communicates treatment plan with those team members that are not available for rounds.

Assists team with administrative tasks

  • Assists with fee tickets completion
  • Assists with portal discharge summary completion
  • Verifies home health orders and directs it to correct Physician.
  • Reviews faxed lab and radiology reports and directs to Physician as needed.

Follow up duties

  • Reassess patients to assure discharge criteria are met to expedite discharge of patient.
  • Schedules follow up tests and inform families of discharge plans.
  • Follow labs that are pending at discharge and inform PCP and family of results.
  • Provide an additional education resource for families.
  • Calls families after discharge, as warranted, to assure medical needs are met and answer additional questions.

Other duties as assigned.

Qualifications

Required Qualifications

  • Bachelor of Science Nursing
  • Requires Current Kentucky Nursing License
  • Minimum of 2 years pediatric nursing experience.
