Overview

We are hiring for a Spinal Cord Injury Nurse Navigator at Frazier Rehabilitation Institute
Location: 220 Abraham Flexner Way, Louisville, KY 40202
Shift: 1st shift Full Time

 

Responsibilities

The Rehab Nurse Navigator provides, supports, and coordinates a comprehensive rehab care continuum at UofL Health - Frazier Rehab. The Rehab Nurse Navigator is an integral part of the rehab team for patients prior to inpatient admissions and beyond outpatient rehab discharge. The Rehab Nurse Navigator provides patient care, education, documentation, and facilitates communication amongst patients, support systems, UofL Health team members, and physicians. The Rehab Nurse Navigator tracks performance improvement measures for specialty populations including patient experience initiatives and quality care metrics and demonstrates a commitment to improving the rehab services provided.

 

A focus of the Rehab Nurse Navigator is to promote patient and staff education by providing group education opportunities and competency training sessions. A Rehab Nurse Navigator will facilitate the development, coordination, and implementation of a patient’s plan of care. It is the responsibility of the Rehab Nurse Navigator to promote rehab staff development and competency completion in the delivery of care to patients with specific diagnoses requiring rehabilitation and serves as a key liaison with clinical educators. The Rehab Nurse Navigator ensures appropriate equipment, supplies, and proper training is available to support the clinical team and the specialty population. The Rehab Nurse Navigator facilitates plan of care management of for patients throughout the continuum of care and collaborates with other members of the care team as needed to optimize patient participation and outcomes.

Qualifications

Education:

  • Associates degrees in nursing (required)
  • Bachelors degree in nursing (preferred)

Experience:

  • 2 years experience in nursing (required)

Licensure:

  • Valid Kentucky Nursing License (required)

Certification:

  • CPR/BLS certification (required) and PALS certification (required for the Pediatric Specialty Program)
  • Certified Registered Rehabilitation Nurse – CRRN (preferred)
  • Program-specific certifications (i.e. CPN, CBIS – preferred)
