Responsibilities

Job Summary:

The nurse navigator functions on a multidisciplinary team as an advocate, educator, and communicator for the oncology patient and their families. 

 

Responsibilities

  • Educates patients and families about disease process, treatment options, and potential side effects. 
  • Coordinate patients through the continuum of care in collaboration with the multidisciplinary team. 
  • Serve as the liaison between disciplines and any supportive/community services. 
  • Coordinate weekly multidisciplinary conferences and database tracking analysis for treatment outcome purposes.
  • Compile data, tracks outcomes, and make recommendations for process improvement.
  • Provide referrals/support for follow-up care for patients.
  • Travel to multiple locations with the ULH system as needed.

Qualifications

Required Qualifications

  • Graduation from an accredited/approved school of nursing.
  • Associate Nursing Degree required – BSN required within 2 years of hire.  
  • Current RN license in Kentucky.
  • 2+ years of oncology experience at the bedside.
  • Excellent communication skills and the ability to interact with nurses, physicians, and skilled healthcare professionals.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • Ability to work autonomously without direct supervision.  

Preferred Qualifications

  • Oncology certified nurse (OCN) recommended or to be obtained within 2 years of employment.
  • Academy of Oncology Nurse and Patient Navigator certification recommended or to be obtained within 2 years of employment.
  • Previous Cerner experience.
