Peace Hospital, part of UofL Health, is a private, non-profit behavioral health hospital. One of the largest behavioral health facilities in the nation, Peace Hospital is known for providing care for children, adolescents, adults and senior adults, age 55 and older.

It’s Kosair Charities Children’s Peace Center is the largest and most comprehensive private provider of youth inpatient behavioral health services in the country. The hospital was the nation’s first behavioral health care center to open a retail pharmacist operated, long-acting injection clinic. The clinic was established to help people adhere to prescribed medications for behavioral health conditions, such as substance use and psychotic disorders.

Job Summary

The Case Manager identifies patient biopsychosocial needs and connects patient with appropriate community resources. The Case Manager interacts directly with patient and guardians to participate in the discharge planning as per the Treatment Plan goals/interventions. They will also coordinate referrals and discharge appointments and collaborates with other service providers and community resources as appropriate.

Qualifications

Required Qualifications

  • Bachelor’s degree in counseling psychology, psychology, or other closely related health degree
  • 18 months experience providing substance use, mental health, or co-occurring treatment services in a behavioral health or related human services setting
  • Ability to complete Crisis Prevention Intervention (CPI) training and successful completion during new hire orientation.
  • BLS required from American Heart Association
