Seeking a physical therapist for a collaborative, multi-disciplinary outpatient department to evaluate and treat orthopedic and neurologic diagnoses. Opportunities available for treatment or training for pelvic health, lymphedema, and/or vestibular diagnoses if the job candidate has the skills and/or interest in training in these specialties. 

Job Summary

The physical therapist is responsible for the evaluation and treatment of patients. In this role you will perform patient evaluation, development and implementation of an appropriate treatment plan, delivery of treatment services, and comprehensive discharge planning. 

Education / Accreditation / Licensure (required & preferred):

• BS/MS/DPT degree in physical therapy required.
• DPT degree preferred.
• Valid PT License Kentucky State Board of Physical Therapy

Certification

• CPR, BLS through the American Heart Association.

Experience (required and preferred):

• Experience in physical therapy preferred for outpatient positions.
• Acute care physical therapy experience required for acute care positions.
