Frazier Rehabilitation Institute, part of UofL Health, has been setting the standard of excellence in rehab medicine for nearly 70 years. Through an expansive network of inpatient and outpatient facilities across Kentucky and southern Indiana, Frazier Rehab offers a wide array of services based on a common goal—helping people of all ages with disabilities reach their fullest potential in physical and cognitive functioning. Frazier Rehab Institute services include nationally recognized brain injury, spinal cord, and stroke rehab programs accredited by the Commission on Accreditation for Rehab Facilities (CARF), with research being conducted with the University of Louisville. Frazier Rehab continues to be on the cutting edge with many other rehabilitation services, such as the EMERGE program which is designed to help patients with severe traumatic brain injuries and who are at low levels of consciousness, and Locomotor training for post spinal cord injury patients which allows for weight support standing and walking.

The Rehab Nurse Navigator provides, supports, and coordinates a comprehensive rehab care continuum at UofL Health - Frazier Rehab. The Rehab Nurse Navigator is an integral part of the rehab team for patients prior to inpatient admissions and beyond outpatient rehab discharge. The Rehab Nurse Navigator provides patient care, education, documentation, and facilitates communication amongst patients, support systems, UofL Health team members, and physicians. The Rehab Nurse Navigator tracks performance improvement measures for specialty populations including patient experience initiatives and quality care metrics and demonstrates a commitment to improving the rehab services provided.
A focus of the Rehab Nurse Navigator is to promote patient and staff education by providing group education opportunities and competency training sessions. A Rehab Nurse Navigator will facilitate the development, coordination, and implementation of a patient’s plan of care. It is the responsibility of the Rehab Nurse Navigator to promote rehab staff development and competency completion in the delivery of care to patients with specific diagnoses requiring rehabilitation and serves as a key liaison with clinical educators. The Rehab Nurse Navigator ensures appropriate equipment, supplies, and proper training is available to support the clinical team and the specialty population. The Rehab Nurse Navigator facilitates plan of care management of for patients throughout the continuum of care and collaborates with other members of the care team as needed to optimize patient participation and outcomes.
