Job Title: Medication History Pharmacy Tech

We are Hiring at University Hospital

Location: 530 South Jackson St  Louisville KY 40202

Shift Options: Varied Hours  Full Time

 

Assists with attaining the UofL Health goal of obtaining accurate medication histories within 24 hours of admission. This activity will consist mainly of face to face interviews with patients in the Emergency Department and with patients directly admitted to inpatient units.

Responsibilities

  • Interviews patients in the Emergency Department or inpatient unit to ascertain an accurate home medication list.
  • Utilizes available software to collect home medication list.
  • If the patient is unable to supply the medication list, the following may need to be contacted:
    • Family member or caregiver
    • Physician’s office
    • Mail-order pharmacy
    • Retail pharmacy
    • Extended care facility
    • Another acute care facility
  • Obtains medication allergy information.
  • Appropriately documents the information obtained in the electronic medical record and clinical pharmacy documentation system.
  • Communicates appropriate information as necessary to other healthcare professionals.
  • Shift rotations may be necessary in order to cover needs (e.g., may be asked to fill in at another downtown campus location if short staffed)
  • Other duties as necessary to fulfill the needs of our patients.
  • Shifts vary by position and location.

Qualifications

Education:

  • High School diploma or GED required. Associate degree in Pharmacy preferred. 

 

Experience:

  • Retail pharmacy experience preferred. Hospital experience is a plus.

Certification:

  • PTCB national certification.

Basic Life Support (BLS) preferred
