  • Overview

    The Blood and Marrow Nurse Clinician operates under a clinical case management model to provide and coordinate care to patients during all phases of the treatment and transplant process in the BMT Program. In collaboration with physicians, residents/fellows, patients and families, the Nurse Clinician (NC) gathers data and uses his/her knowledge of complex health problems to provide and facilitate the delivery of quality, evidenced-based care, while promoting efficient and effective patient outcomes in both the inpatient and outpatient setting. This position provides education and support to physicians, staff, patients, and families regarding innovations/evidence for quality outcomes. In collaboration with physicians, staff, patient and families, the NC facilitates implementation of the plan of care during all phases of treatment and facilitates communication among patients/family and all care providers. The NC takes primary responsibility for admission and discharge planning. The NC is cognizant of the psychosocial and financial impact that disease and hospitalization has on the patient and family and acts as an advocate to identify and obtain needed resources and support. Additionally, the NC assists the physician with procedures (including bone marrow harvests and administration of stem cell products,) triages phone calls, rotates in 24 hour on-call responsibilities, and participates in clinical research. The BMT Nurse Clinicians use Nursing Practice Competencies to achieve safe, optimal patient and family centered care health outcomes; Leadership Competencies to coordinate patient care and communicate with the Care Team; Professional Profile Competencies to develop professionally as a life-long learner, and Practice Outcomes Competencies to evaluate care based on evidence, the ULH quality model, and patient safety goals.

    Responsibilities

    1. Facilitates patient and family-centered rounding and interactions, promoting collaboration, and communication between the patient, family, and all health care providers in both the inpatient and outpatient setting. Plans and manages care collaboratively with the interdisciplinary team, patient and family.


    2. Performs daily acute health assessments, including pertinent history, review of systems and physical assessment; collects and reviews all pertinent clinical and diagnostic data to assist physicians and team members to thoroughly assess and evaluate each patient encounter. Assists in interpreting findings with the team and shares findings with patient and family in a meaningful manner. Monitors patient’s response to treatment and works with all health care providers, patient and family to adjust plan of care as needed.


    3. Collaborates with physicians, nursing staff, patient /family, and other disciplines to define plan of care with measurable clinical outcomes. Takes primary responsibility for coordinating the plan of care across the continuum of treatment (from the time the patient enters the BMT program until they return to the care of the referring physician.) Coordinates discharge planning, teaching and follow-up, identifying needed resources to effectively manage patient care in the outpatient setting.


    4. Assists patient and family in navigating healthcare system for clinical needs. In collaboration with physician and patient/family, assesses need for referrals to other healthcare resources and assists with contacts.


    5. Collaborates with patient and family, nursing personnel, and care coordination staff/ social services to assure needs are clearly understood and met (including psychosocial, financial, prescriptions, transportation or other factors that could negatively impact care).


    6. Maintains patient outcome as central focus. (Outcomes include excellent clinical management, appropriate patient and family education, follow-up care, and patient’s perception of satisfaction with services and care provided). Identifies actual and potential problematic situations and actively intervenes to achieve a satisfactory resolution.


    7. Serves as a resource for the patient and family by providing comprehensive education at all phases of care that is adjusted to the learning needs/level of the patient and family. Provides relevant educational materials.


    8. Provides additional emotional support for the patient and family throughout the treatment process, with particular attention to the special problems of the critically ill BMT patient.


    9. Assists the BMT physician during marrow/stem cell harvests and infusions of stem cell products, following appropriate policies and procedures. Completes appropriate documentation needed for bone marrow harvests, peripheral stem cell collections and infusions.


    10. Works as a team with other BMT nurse clinicians and Nurse Practitioners by sharing in clinical responsibilities. Triages all calls from BMT outpatients (and their families), hospital staff, home care agencies, and Hosparus. Collects and synthesizes pertinent data, communicates the need or issue to the BMT physician, and implements plan, following all policies, procedures, or physician-directed protocols. Documents relevant information in medical record. Rotates with clinician team on 24 hour, 7 days/week coverage to assist the BMT physician in patient management.


    11. Serves as resource person to contribute to the training and education of other BMT nurse clinicians, physicians and staff.


    12. Identifies patients who are potentially eligible for research studies and makes referral to the Clinical Trials Office; disseminates protocol information and assists in protocol compliance.


    13. Participates in leadership activities, such as participating in weekly interdisciplinary patient care meetings, assigned committees, TJC preparation, FACT preparation, and quality improvement activities as assigned.


    14. Demonstrates continued professional growth by actively seeking opportunities for continued learning and obtaining/ maintaining specialty certification. Participates in Magnet activities and other hospital initiatives. Active participant in the Family Center Care model.


    15. Performs other duties, within realm of competency, as assigned.

    Qualifications

    EDUCATION & TRAINING: Graduation from an accredited/approved school of nursing. BSN required. Licensed to practice professional nursing in the State of Kentucky.


    EXPERIENCE: Three years nursing experience required. Two years of oncology nursing experience preferred. Critical care and/or blood and marrow transplantation experience preferred.


    LICENSE & CERTIFICATION: Current RN license in Kentucky. Current AHA certification in Basic Life Support (BLS). OCN certification required within 2 years of hire.
