    Develop and strengthen contacts with EMS and referral facilities. Identify and develop additional

    referral markets in stroke in cooperation with Trauma and Cancer Service Lines.

     

    Design and implement stroke-related education and screening programs for community

    presentation.

     

    Design and implement educational programs and support programs for referring physicians,

    referral facilities and EMS.

     

    Design and implement an effective primary care, referral source and EMS

    communication/feedback program to enhance continuity of care and strengthen referring

    relationships.

     

    Recruit and supervise volunteer and employed personnel in executing outreach programs. 

     

    Supervise administrative assistant in planning, implementing and tracking executive outreach

    strategies.

     

    Responsibilities

    * Develops and leads cohesive strategy with Stroke Institute Director and Medical Director in assessing

    community, EMS and outlying facility referral development opportunities.

     

    * Based on identified opportunities and innovative strategies implemented, broaden ULH’s stroke referral

    base relationships such that stroke referrals increase year over year by no less than 5%.

     

    * Serves as a resource for outlying facility education/prevention and quality improvement; assists with

    program development as requested.

     

    * Collaborates with program assistant, APRNs, physicians and Stroke Resource Nurses to provide follow-up

    contact with referring physicians/facilities, primary care and EMS as indicated through written and verbal

    communication.

     

    * Establish, lead and execute an annual calendar of “signature” events sponsored by UL Comprehensive

    Stroke Center.

     

    * Track and document all internal and external outreach events undertaken by the stroke program as

    documentation for TJC survey and in ULH sales management database.

     

    * Analyze trends in outreach offerings for efficacy in supporting the stroke program and prioritize

    participation in those yielding the greatest reward for our program while respecting clinician time.

     

    * Establish and execute a system for providing feedback to PCPs and other relevant stakeholders in our

    patient’s care on a timely basis.

     

    * Establish, lead and execute a multi-pronged EMS outreach program 

     

    * Attend and support KBEMS Board meetings and initiatives.

    * Establish a facility-wide team to work collaboratively with UL trauma outreach and other elements of internal outreach to coordinate efforts so as not to inundate the customers with multiple UL contacts and to maximize our outreach impact and efficiency across the organization.

     

    * Engage direct care staff as well as physicians and program leadership in outreach efforts to increase their sense of ownership in the stroke program.

     

    * Provide report on outreach efforts at stroke administrative meetings on a routine basis. 

     

    * Develop and implement University of Louisville Hospital Speakers Bureau for outside facility continuing education (pre-hospital, physician, nursing)

     

    * Track stroke referrals and maintain related database.

     

    * Develop, promote and present stroke prevention community programs based on needs assessment in keeping with Joint Commission Comprehensive Stroke Center requirements.

     

    * Be actively involved in hospital sponsored stroke related community programs

     

    * Member of Stroke Executive and Operational Committees.

     

    * Stays informed regarding quality improvement programs related to stroke patient care and monitor/report outcomes to external stakeholders as appropriate.

     

    * Serves on stroke- related hospital committees and participates in outside committees as requested.

     

    Qualifications

    Education / Accreditation / Licensure (required & preferred): 

     

    * Bachelor’s degree in health-related topic or business mandatory, Health- related masters preferred. 

    * RN preferred

    Experience (required and preferred): 

     

    * Minimum three years program management required. 

    * Public speaking experience essential and required

    * Business development experience essential and required
