The Oncology Rehabilitation nurse navigator functions amongst and within all Oncology multidisciplinary
teams as an advocate, educator, and communicator for the oncology patient and their families regarding
cancer rehabilitation needs. The navigator ensures that all patients with an oncology diagnosis receive
quality rehabilitation care across the growing Brown Cancer Center and Frazier systems. Educates
patients and families about rehabilitation needs for cancer patients and how to maximize functional
capacity. Literature states >65% cancer patients have rehabilitation needs, and the goal is to help
identify these needs and provide access to rehabilitation. He/she will coordinate patients through the
continuum of care (prehabilitation, supportive and restorative rehabilitation, and palliative rehabilitation)
in collaboration with the Brown Cancer Center multidisciplinary teams and Physical Medicine and
Rehabilitation clinics. He/she will serve as the liaison between disciplines and any supportive/community
services. He/she will provide leadership within the team, proactively develop processes that streamline
and improve the clinical experience for both the patient and referring physicians, and track cancer
rehabilitation growth across the system. He/she will work closely with marketing and outreach
departments to educate referring physicians and facilities on available rehabilitation services. He/she will
be involved in coordinating rehabilitation care from inpatient to outpatient to home health for patients.
He/She will be engaged with all Cancer Rehabilitation clinics and remain available to patients and
families for questions after clinic and inpatient visits. He/she will demonstrate the knowledge, skill and
coordination to provide the care and guidance to the cancer patient through their rehabilitation needs.

The MEDICAL EMERGENCY RESPONSE INTERVENTIONAL TEAM (MERIT) staffed by highly experienced ICU nurses with diverse specialty backgrounds who use technology, innovation, and education to improve critical care and quality outcomes. The MERIT nurse responds to and leads clinical bedside emergencies throughout the campus. Partner with the Resource Intensivist or designee to optimize code response in terms of efficiency and to decrease incidence of inpatient hospital mortality. Leads debriefing with staff after a code is called. Mentors staff needing guidance in nursing care related to ACLS and the first five minutes. Provides follow up with patients transferred out of the ICU to reduce unplanned returns to the ICU. The MERIT nurse also streamlines the post-mortem process. The MERIT nurse may complete various audits of unit practices and compliance measures. Performs other duties as assigned within the scope of nursing practice.
This position is accountable for the daily patient care operations of the assigned department, working under the direction of the Nurse Manager on assigned shifts. Responsible for daily patient care assignments, maintaining clinical standards of care, maintaining labor productivity and supervision of the patient care environment; supporting the care team. Supports the activities of the Nurse Manager. Maintains positive inter- and intra-departmental relationships. Directs patient throughput activities in the assigned unit. Maintains evidence-based nursing practices to achieve desirable patient outcomes in accordance with the Kentucky Nurse Practice Act.
