A focus of the Rehab Nurse Navigator is to promote patient and staff education by providing group education opportunities and competency training sessions. A Rehab Nurse Navigator will facilitate the development, coordination, and implementation of a patient’s plan of care. It is the responsibility of the Rehab Nurse Navigator to promote rehab staff development and competency completion in the delivery of care to patients with specific diagnoses requiring rehabilitation and serves as a key liaison with clinical educators.
